This agreement applies to the participant named in this record. If the participant is under 18, the undersigned parent or legal guardian is signing on the participant’s behalf.
I understand that Taekwondo and other martial-arts activities—including introductory classes, instruction, drills, practice, demonstrations, testing, competition, and related KTA activities—involve physical activity and contact. Injuries may occur and may range from minor to serious or catastrophic. I voluntarily choose to participate, or permit my child to participate, with this understanding.
I confirm that I, or my child, am able to participate in the activity. I will disclose relevant medical conditions or limitations to KTA staff and will follow KTA instructors’ and staff members’ safety directions, rules, and procedures.
I authorize KTA instructors, staff members, and representatives to provide reasonable temporary first aid in the event of illness or injury. If they reasonably believe emergency medical care or ambulance transport is needed, I authorize them to seek or arrange that care.
To the fullest extent permitted by law, I release Korea Taekwondo Academy, Inc. (KTA), its instructors, staff members, and representatives from claims arising from injury or loss connected with the participant’s involvement in KTA activities, except for responsibilities that cannot lawfully be waived.
I affirm that I have, and will continue to maintain, health and accident insurance coverage that I consider adequate for myself or my child.
This agreement applies to this introductory class and future KTA activities attended under this participant record unless KTA replaces it with a later written agreement or it is otherwise revoked in writing.
By signing, I confirm that I have read and understand this agreement, have had the opportunity to ask questions, and agree to it voluntarily. I am at least 18 years old and am the participant or the participant’s parent or legal guardian.
